Release, Waiver of Liability, and Covenant Not to Sue
In agreeing to participate myself, and/or to register and allow any child, dependent, or family member
listed in the registration form to attend Muslim Association of Canada (MAC) activities and/or
programs, I understand that MAC will not be liable for any loss, injury, or death resulting from any
risks outlined herein. I agree to waive my right, and/or the rights of any individuals I am registering, to
sue MAC for any loss, injury, or death resulting from the risks outlined within this agreement.
For and in consideration of MAC permitting me and/or any child, dependent, or family member I
register, collectively referred to as the “Registered Participant(s),” to join any MAC activities, camps,
programs, and to participate in any activity of any nature conducted thereon, I hereby:
1 Waive, release, and discharge any and all claims, demands, actions, or causes of action I may
have personally, and/or on behalf of any Registered Participant(s), for injury, property damage, or
death arising from participation in MAC activities, whether caused in whole or in part by the
negligence of MAC, its employees, volunteers, members, or other participants.
2 Assume full responsibility for any risk of personal injury, property damage, or death to myself
and/or any Registered Participant(s), including risks arising from negligence while participating in
MAC activities on the premises or elsewhere.
3 Covenant not to sue MAC, its employees, volunteers, or members on my own behalf and/or on
behalf of any Registered Participant(s), for any injury, loss, or damage sustained as a result of
participation in such activities.
4 Confirm voluntary participation, declaring that I am willingly participating and/or allowing the
Registered Participant(s) to participate, and I have voluntarily agreed to sign this Release, Waiver
of Liability, and Covenant Not to Sue.
5 Express clear intent to release liability, acknowledging that by signing this agreement, I release
MAC, its employees, volunteers, and members from any and all liability for personal injury,
property damage, or death caused by negligence to myself and/or any Registered Participant(s).
I acknowledge and agree that this Agreement is valid from the date I complete the online registration
form until the end of the MAC activity and/or program as stated in the registration.
