Ramadan FoodShare Packing Event 2025

I declare that I am the legal parent/guardian of the above-named child(ren) and give my permission for my child(ren) to participate in the MAC Youth Jr. events. I understand that this authorization to allow my child(ren) to participate in the MAC Youth Jr. events is a waiver of all claims that I, my child(ren), or other family member(s), or my insurance carrier would have against MAC, its board, employees, or program staff. I agree that I am responsible for the accuracy of information entered and the registration will be considered cancelled if I have provided mistaken information. In that case, I will not be issued any refund for payment I have made.

I agree that in the case that my child is injured or becomes ill and I cannot be reached, I give my consent to initiate medical treatment as deemed appropriate by the program administration and/or an attending physician. If in the event that my child requires special medication, X­-ray or treatment beyond that which is possible at event, I will cover any additional expenses at cost.

I agree to allow my child to participate in all the program activities. I agree that the program administration reserves the right to dismiss my child when it is deemed to be in the best interest of both my child and the program. I agree that there will be no deductions or refunds for participants arriving late or leaving before the end of the program. I agree that in the event that my child is required to leave event early that it is my responsibility to arrange necessary transportation. I consent to the use, by the MAC, of the participant’s photos for marketing and publicity purposes (i.e. posted on social media, MAC website). I agree that MAC is not responsible for any damages or loss of personal possessions while at camp or while participating in any camp activity.

I hereby release the Muslim Association of Canada (MAC) and the MAC Youth Jr events Staff from all claims from damages arising from any accident or injury, which may result from such participation of my child (named above) during any program in any facility where the program is held. I have reviewed the above requirements for participation with my child and we agree that I will be responsible for my child’s actions.
