If an age group is sold out, please DO NOT register your child in the wrong group.
Send an email to: youth.jr@mississauga.macnet.ca requesting that your child be added to the waitlist.
Provide: Child First and Last Name, Grade, Parent Phone number.

We will always do our best to accommodate the children on the waitlist.

If your register your child in the wrong age, we will not be accommodating them, and no refund will be provided. Thank you for your understanding.

I declare that I am the legal parent/guardian of the above-named child(ren) and give my permission for my child(ren) to participate in the MYJ R4R Camp at Camp Wanakita.

I understand that this authorization to allow my child(ren) to participate in the MYJ R4R Camp is a waiver of all claims that I, my child(ren), or other family member(s), or my insurance carrier would have against MAC, its board, employees, or program staff.

I agree that I am responsible for the accuracy of information entered and the registration will be considered canceled if I have provided mistaken information. In that case, I will not be issued any refund for the payment I have made.

I agree that in the case that my child is injured or becomes ill and I cannot be reached, I give my consent to initiate medical treatment as deemed appropriate by the camp administration and/or an attending physician. If in the event that my child requires special medication, X­-ray or treatment beyond that which is possible at camp, I will cover any additional expenses at cost.

I agree to allow my child to participate in all the camp activities. If my child requires any exemption, I will provide a clear written request. I agree that the MYJ R4R Camp administration reserves the right to dismiss my child when it is deemed to be in the best interest of both my child and the Camp. I agree that in the event that my child is required to leave camp early that it is my responsibility to arrange necessary transportation.

I consent to the use, by MAC, of the camper’s photos for marketing and publicity purposes (i.e. posted on social media, MAC website, etc.)
I agree that MAC is not responsible for any damages or loss of personal possessions while at camp or while participating in any camp activity.

I hereby release the Muslim Association of Canada (MAC) and the MYJ R4R Camp Staff from all claims from damages arising from any accident or injury, which may result from such participation of my child (named above) during any program in any facility where the program is held.

Parents / Guardians of the camper MUST attend the Parent info session, planned for Saturday February 15th, 2025.
